After a year, we’re still grappling with the COVID-19 virus, but now we could be more hopeful with the continuous rollout of vaccines. To make the process easier and safer, a number of local government units have opened the registration for their vaccination programs online.
On this list, we’ve rounded up the LGUs and the links to their online registration forms so you can get your vaccine soon!
Manila City
Register at http://manilacovid19vaccine.com. Just fill out the form and click “Validate”. Enter the OTP (one-time passcode) number that will be sent to your mobile number to confirm and click “Register”. Click the “I Agree” button and affix your signature on the waiver page, and you’re done!
Makati City
To register, go to https://www.covid19vaccine.safemakati.com. Choose which sector you belong to and provide the necessary details such as your ID number to confirm. Fill out the rest of the form and you will be given your confirmation number to track your form. You can also update it through the Search button.
Quezon City
Quezon City Mayor Joy Belmonte said in January that their unified QCitizen ID system will also be used for the rollout of COVID-19 vaccines. Register here: https://qceservices.quezoncity.gov.ph. Meanwhile, active health workers are also encouraged to register through https://app.ezconsult.io/signup. Click here for more information.
Caloocan City
Register through this link: bit.ly/profilingcalv2. You will be greeted with a list of sectors arranged according to which ones will be prioritized. Select the sector you are in and fill out the form.
Las PiƱas City
Go to this link: https://bit.ly/LasPiƱasVaccination, fill out the form with your personal details including your ID information, and submit the form.

Mandaluyong City
Registration is at www.mandaluyong.gov.ph/vaccine/. Fill out the online registration form, upload a proper ID picture, click “I agree to the terms, conditions, and data privacy” and submit the form. You can then get your temporary vaccination card and waiver. Bring these on the day of your vaccination.
Muntinlupa City
Just go to this link: https://vaccine.muntinlupacity.gov.ph/muncovac/, and enter the needed details on the form, including your preferred vaccination area, and submit the form.

Valenzuela City
To register, go to this link: valtrace.appcase.net and click “Vaccination Registration”, and click “Nauunawaan ko/ I understand”. Fill out the form and upload a photo of your ID, and submit. Once approved, you will receive a phone call or email with your vaccination schedule.
